Effectiveness of a Health Literacy Development Program on Blood Pressure Control among Patients with Uncontrolled Hypertension: A Quasi-experimental Study at Saphandam Community Health Center, Sawanpracharak Hospital

Objective: To evaluate the effectiveness of a health literacy development program on blood pressure, hypertension-related knowledge, and health behaviors among patients with uncontrolled hypertension.
Methods: A quasi-experimental study with a comparison group was conducted among 164 patients with uncontrolled hypertension. Participants were allocated to an experimental group (n = 82) and a comparison group (n = 82). The experimental group received a health literacy development program consisting of health education, continuous follow-up through a LINE group, and at least one home visit during the 20-week study period. Outcomes were assessed at baseline, week 10, and week 20. Data were analyzed using descriptive statistics and independent t-test. Statistical significance was set at p-value < 0.05.
Results: At baseline, there were no significant differences in mean systolic or diastolic blood pressure between the two groups (p-value = 0.18; and p-value = 0.54, respectively). Following the intervention, the experimental group had significantly lower systolic and diastolic blood pressure than the comparison group at both weeks 10 and 20 (all p-value < 0.001). Hypertension-related knowledge scores did not differ significantly at baseline or week 10 (p-value = 0.92 and p-value = 0.07) but were significantly higher in the experimental group at week 20 (12.9 ± 2.0 vs. 10.8 ± 2.5, p-value < 0.001). At week 20, the experimental group also demonstrated significantly higher health literacy scores (42.8 ± 3.6 vs. 35.9 ± 4.5, p-value < 0.001) and health behavior scores (42.1 ± 6.4 vs. 39.8 ± 4.8, p-value < 0.001) than the comparison group.
Conclusions: The health literacy development program effectively reduced systolic and diastolic blood pressure and improved hypertension-related knowledge, health literacy, and health behaviors among patients with uncontrolled hypertension. The program should be implemented in primary care settings, and long-term follow-up studies are recommended to confirm the sustainability of its effects.
