ความชุกและปัจจัยที่สัมพันธ์กับความผิดปกติของการนอนหลับเนื่องมาจากการทำงานเป็นกะในพยาบาลสังกัดโรงพยาบาลขนาดใหญ่แห่งหนึ่ง

บทคัดย่อ

การทำงานเป็นกะทำให้ระบบภายในของร่างกายทำงานไม่สอดคล้องกับนาฬิกาชีวภาพ และอาจนำไปสู่ความผิดปกติของการนอนหลับเนื่องจากการทำงานเป็นกะ (shift work disorder) งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาความชุกและปัจจัยที่สัมพันธ์กับความผิดปกติของการนอนหลับเนื่องจากการทำงานเป็นกะในพยาบาลสังกัดโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย โดยเป็นการศึกษาเชิงพรรณนาแบบภาคตัดขวางในพยาบาลจำนวน 384 คน ที่ได้จากการสุ่มตัวอย่างแบบกลุ่มภายใต้ชั้นภูมิ เก็บข้อมูลโดยใช้แบบสอบถามประกอบด้วย ข้อมูลส่วนบุคคล แบบประเมินภาระงาน (NASA-TLX) และแบบคัดกรอง Shift Work Disorder Index (SWDI) วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและสถิติ multiple logistic regression ผลการศึกษาพบความชุกของ shift work disorder ในพยาบาลที่ทำงานเป็นกะ ร้อยละ 60.7 และคิดเป็นความชุก ร้อยละ 53.1 ในพยาบาลทั้งหมด ปัจจัยที่มีความสัมพันธ์กับ shift work disorder อย่างมีนัยสำคัญทางสถิติ ได้แก่ การมีจำนวนกะดึกเฉลี่ยมากกว่า 5 กะต่อเดือน (AOR=3.66; 95%CI: 1.89-7.07; p<0.001) การกลับเข้ากะเร็ว (quick return) เฉลี่ย 1 ถึง 5 ครั้งต่อเดือน (AOR=2.66; 95%CI: 1.17-6.04; p=0.019) การกลับเข้ากะเร็วเฉลี่ยมากกว่า 5 ครั้งต่อเดือน (AOR=4.48; 95%CI: 1.94-10.35; p<0.001) และการรับรู้ภาระงานระดับสูง (AOR=2.54; 95%CI: 1.51-4.26; p<0.001) ในขณะที่การปฏิบัติงานในแผนกอุบัติเหตุฉุกเฉินเป็นปัจจัยป้องกันเมื่อเทียบกับหอผู้ป่วยใน (AOR=0.20; 95%CI: 0.07-0.58; p=0.003) ผลการวิจัยนี้บ่งชี้ว่า องค์กรมีขนาดปัญหา shift work disorder ในพยาบาลที่สูง โดยปัจจัยที่เกี่ยวข้องกับงาน ได้แก่ จำนวนกะดึก การกลับเข้ากะเร็ว และภาระงานที่สูงเป็นปัจจัยเสี่ยงสำคัญที่สามารถปรับเปลี่ยนได้ องค์กรควรพิจารณากำหนดนโยบายที่เกี่ยวข้องกับปัจจัยดังกล่าวเพื่อสุขภาวะของพยาบาล
