การปลูกถ่ายฟันให้ตนเองในผู้ป่วยจัดฟัน

การปลูกถ่ายฟัน ให้ตนเอง( tooth autotransplantation) เป็นทางเลือกหนึ่งที่ใช้รักษาทดแทนช่องว่างของฟันที่ขาดหายไป การศึกษานี้เป็นการศึกษาย้อนหลังมีวัตถุประสงค์เพื่อศึกษาอัตราความสำเร็จ และปัจจัยที่เกี่ยวข้องกับความสำเร็จของการปลูกถ่ายฟันในผู้ป่วยที่ได้รับการจัดฟันร่วมด้วย โดยศึกษาจากบันทึกเวชระเบียนและภาพถ่ายรังสีในผู้ป่วยที่ได้รับการปลูกถ่ายฟันให้ตนเองอย่างน้อย1ซี่ที่ผ่าตัดด้วยทันตแพทย์คนเดียวกันร่วมกับการจัดฟันที่ศิริเพ็ญเมโมเรียลทันตคลินิก มีระยะเวลาติดตามผลการรักษาไม่น้อยกว่า 1ปีขึ้นไปโดยมีเกณฑ์วัดความสำเร็จคือฟันที่ปลูกถ่ายยังคงอยู่ มีเหงือกโดยรอบปกติ ฟันไม่โยก ไม่มีankylosis ไม่มีการละลายตัวของรากฟัน รากฟันไม่สั้นโดยมีอัตราส่วนความยาวของ ตัวฟันและรากฟันต้องไม่น้อยกว่า1 พบว่ามีผู้ป่วย 27 รายเป็นเพศชาย 9ราย(ร้อยละ33.3)เพศหญิง 18ราย(ร้อยละ66.7)อายุตั้งแต่9ถึง 23 ปีอายุเฉลี่ย 16 ปีได้รับการปลูกถ่ายฟันให้ตนเองจำนวน30ซี่ เป็นฟันที่ปลายรากยังไม่ปิด 24 ซี่(ร้อยละ80) ปลายรากปิดแล้ว6ซี่(ร้อยละ20) ระยะเวลาติดตามผลการรักษาตั้งแต่ 1.5-30 ปีเฉลี่ย 6.8 ปี ผลการศึกษาพบว่ามีฟัน1ซี่ถูกถอน(ร้อยละ3.3) และฟัน2ซี่เกิดankylosis(ร้อยละ6.7) พบมีอัตราความสำเร็จอยู่ที่ร้อยละ90
สรุปการปลูกถ่ายฟันให้ตนเองในผู้ป่วยที่ได้รับการจัดฟันร่วมด้วย ประสบความสำเร็จ ในอัตราที่สูงหากเลือกฟันที่เหมาะสมและใช้เทคนิคการผ่าตัดที่ดี วีธีนี้จึงควรพิจารณาเป็นทางเลือกที่ใช้รักษาทดแทนฟันที่สูญเสียไปโดยเฉพาะอย่างยิ่งในผู้ป่วยที่ยังมีการเจริญเติบโตของกระดูก
